
Clinical Pharmacists
Provide direct patient care throughout the institution. Clinical pharmacists are active, integral members of medical service teams and assist the team by providing services including:
- Drug selection and dosing
- Pharmacokinetic assessment and drug dosing
- Anticoagulation dosing and monitoring
- Antibiotic screening and monitoring
- Parenteral nutrition dosing and monitoring
- Drug information
- Patient medication counseling and education
- Outpatient anticoagulation clinic
- Other patient-focused services to optimize drug therapy outcomes

Standards of Practice and Uniform Level of Care
All pharmacists must be graduates of an accredited college of pharmacy and meet all requirements identified in the Indiana Pharmacy Practice Act. Pharmacy Services at Monroe Hospital follows standards of practice as identified by the following organizations:
- American Society of Health-System Pharmacists
- American Pharmaceutical Association
- The Joint Commission
- Indiana Board of Pharmacy (also, Medicine, Nursing, etc.)
- Food and Drug Administration
- Drug Enforcement Agency
